Question: biceps brachii tendon injury (or not)?

What do I do when there is pain localized to the long head of biceps brachii a day after intense training in chin ups (both close grip and wide) and full ROM in the glenohumeral joint 3 weeks post injury, with continued pain at the tendon site yet no pain on resisted elbow flexion nor resisted supination? Has anyone every had pain referred to this very specific 2 inch strip over the tendon WITHOUT injury to biceps brachii? Cervical region is clear and my patient otherwise very fit and healthy. Strength testing and resisted ROM on all shoulder muscles seem fine, with mild soreness in coracobrachialis on palpation. Weakness and pain at 45 degrees forward flexion and around 45 degrees abduction yet supraspinatus and deltoid pain free on palpation. All ideas, suggestions welcome!

Answer:

Try a Speed's test and a Yergason test and palpate for any tendon subluxation of the biceps. Also remember that coricobrachialis runs deep to the short head of biceps and that latissumus dori inserts just medial to the bicepital groove.
