Question: How can you determine what kind of shoulder injury you have sustained with exercise?

I believe I may have pulled or strained (is that same thing?) my shoulder when doing upper body work out. The pain is not consistent and hurts more when I've used the shoulder too much. The pain is on the backside of the shoulder and sometimes depending on what I'm doing, the pain extends to the back of shoulder blade to elbow as well. It's been well over a week and does not seem to be getting better. I'm really concerned I've incurred a serious injury. I can lift my arm over my head and can sleep on shoulder ok, although I try to avoid that as much as possible. It's not excruciating pain but painful nonetheless. Getting pretty frustrating and not sure if serious enough to see a physician yet. Any advice would be helpful.
